If you have not been tattooed here before and you have a scheduled tattoo appointment, please fill out the form linked above. This must be done prior to getting tattooed.

You still need to bring your ID to your tattoo appointment so that it can be checked.

This form is only for minors who would like to get pierced. If you are over 18, you do not need this.

In addition to this form, the parent/legal guardian of the minor must have a valid ID and be present during the time of the piercing. We ask that only the person being pierced & the parent/guardian come in for the piercing appointment due to COVID-19. Thank you for understanding as we navigate this new way of operating.
